Job Summary

The Mental Health Social Worker is a key member of the Mental Health Team at Southlake Regional Health Centre. As a social worker, you will be responsible for providing high-quality patient care and support to individuals with serious mental illness. Your expertise will be essential in assessing, treating, and counseling clients, as well as leading group therapy sessions and collaborating with other healthcare professionals.

Primary Responsibilities
  • Assess and treat clients with serious mental illness in the Adult Outpatient Mental Health program
  • Develop and implement treatment plans, including short-term counseling and psycho-social work
  • Lead group therapy sessions and facilitate group dynamics
  • Document clinical assessments, treatment plans, and client progress
  • Liaise with team members, hospital community, and community agencies to promote good working relationships
  • Provide education to the community and hospital
  • Collect statistical data and participate in committee membership as required
Qualifications
  • Masters in Social Work (MSW)
  • Three to five years experience working with seriously mentally ill clients
  • Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Advanced counselling and assessment skills with various short-term counselling modalities
  • Demonstrated ability to work collaboratively in an interprofessional team environment
  • Regular attendance and commitment to customer service and teamwork
