About the Role

We are seeking a highly skilled and compassionate Mental Health Social Worker to join our team at Southlake Regional Health Centre. As a key member of our interdisciplinary team, you will provide assessment, support, and counseling services to individuals and their families on our inpatient mental health unit.

Key Responsibilities
  • Conduct individual, couple, and family assessments to determine the type of services required for successful discharge
  • Provide short-term, supportive counseling and education to clients and their families
  • Support clients with the discharge planning process, including referrals to community resources and housing programs
  • Facilitate daily inpatient rounds with participating community partners and liaise with community partners as required
  • Document assessments, treatment plans, and clinical progress notes
Qualifications

To be successful in this role, you will require:

  • A Master's degree in Social Work (MSW)
  • Three to five years' experience working with seriously mentally ill clients
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Advanced counseling and assessment skills, including Brief Solution Focused model, cognitive behavioral, and dialectical behavioral therapy
  • Ability to work collaboratively in an inter-professional team environment and independently
