We are seeking a highly skilled and compassionate Social Worker to join our Inpatient Mental Health team at Southlake Regional Health Centre. As a key member of our interdisciplinary team, you will provide assessment, support, and counseling services to individuals and their families, promoting optimal mental health outcomes and facilitating successful discharge planning.
Key Responsibilities- Conduct comprehensive assessments of patients' mental health needs, identifying areas for intervention and developing individualized treatment plans.
- Provide short-term, supportive counseling and education to patients and their families, promoting coping skills and resilience.
- Collaborate with the interdisciplinary team to develop and implement discharge plans, ensuring seamless transitions to community-based services.
- Facilitate referrals to community resources, income support programs, and housing services, as needed.
- Lead and facilitate psychoeducational and life skills groups, promoting patient engagement and empowerment.
- Document assessments, treatment plans, and clinical progress notes, ensuring accurate and timely reporting.
- Participate in daily inpatient rounds, collaborating with community partners to promote patient care and well-being.
- Liaise with community partners to discuss program changes, patient care needs, and promote good working relationships.
- Support patients and the treatment team through group programming, promoting a culture of respect and inclusivity.
- Collect and analyze statistical data, contributing to the development of evidence-based practice.
To be successful in this role, you will possess:
- A Master's degree in Social Work (MSW) from an accredited program.
- Three to five years of experience working with seriously mentally ill clients, with a strong understanding of mental health disorders and treatment modalities.
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Advanced counseling and assessment skills, with experience in Brief Solution Focused model, cognitive behavioral, and dialectical behavioral therapy.
- Demonstrated ability to work collaboratively in an inter-professional team environment, as well as independently.
- Ability to de-escalate crisis situations, with a strong commitment to patient safety and well-being.
- Good working knowledge of Social Assistance forms and hospital informational systems.
- Knowledge of clinical practices, community resources, and income support programs.
- Advanced group facilitation skills, with experience leading psychoeducational and life skills groups.
- Ability to work with individuals and their families, promoting patient-centered care and empowerment.
- Advanced counseling skills, including CBT, Solution Focused, and Suicide Prevention Skills.
- SMG Training and understanding of the Mental Health Act.
- Excellent communication, analytical, and problem-solving skills, with a strong commitment to customer service and teamwork.
