Key Responsibilities- Assess and Determine Patient Care Needs: Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans.
- Link Patients with Service Providers: Connect patients with service providers, ensuring seamless access to community resources.
- Coordinate and Monitor Care Plan Delivery: Oversee the implementation and monitoring of care plans, ensuring timely and effective delivery of services.
- Establish a Helping Relationship: Foster a supportive relationship with patients and their families, prioritizing their needs and preferences.
- Balance Patient Needs and Resources: Effectively balance patient needs with available resources, ensuring that patients' values and preferences are respected.
- Membership with Regulatory Body: Hold membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
- Experience in Community Health: Possess 2+ years of recent experience in community health or a related field.
- Knowledge of Healthcare Delivery System: Demonstrate knowledge of the healthcare delivery system and community resources.
- Excellent Interpersonal Skills: Exhibit excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
- Effective Time Management: Possess effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment.
- Accurate Documentation: Demonstrate established ability to accurately complete required documentation, reports, and forms.
- Valid Driver's Licence: Hold a valid driver's licence and have access to a reliable vehicle.
- Proficient in Windows Environment: Be proficient in a Windows environment.
- Experience with Diverse Patient Groups: Possess experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
- Case Management Experience: Have experience in case management or recent related community experience.
- Language Skills: Possess the ability to speak French or another second language.
