We are seeking a skilled and compassionate Case Manager to join our team at The Salvation Army. As a Case Manager, you will play a vital role in providing support services to residents of The Gateway Shelter, promoting housing stability and overall well-being.
Key Responsibilities- Provide direct consultation, referral, and supportive case management to residents of the shelter.
- Conduct intake interviews with first-time and ongoing residents to determine needs and offer appropriate supports and referrals.
- Develop and implement case plans to help residents achieve their goals and secure stable housing.
- Collaborate with community resources to provide residents with access to essential services and support.
- Document all relevant case notes and maintain confidentiality and accuracy of case files.
- Participate in regular case management meetings and other meetings as required.
This is a full-time position based on 40 hours per week, Monday to Friday. The successful candidate will work in an emergency housing facility, subject to high client levels of stress and crisis intervention will be required.
Qualifications and Education Requirements- 2 years of College Social Services education and/or Psychology diploma or certificate.
- Certificate in First Aid/CPR, Crisis Intervention, and Conflict Resolution.
- Valid Ontario Class "G" Driver's License and a current copy of a Driver's abstract for review and verification that is satisfactory to The Salvation Army.
- Minimum of two-year experience in the field of Mental Health or Addictions.
- Good knowledge of community resources.
- One to three years of prior related experience, including frontline casework experience in a social service environment.
- Excellent oral and written communication skills (computer skills essential).
- Well-developed skills and ability to work independently as well as part of a team.
- Proven ability to constructively give and receive feedback.
- Highly developed understanding of and proven ability to communicate appropriate professional boundaries.
