About the Opportunity

We are seeking a skilled Advance Care Planning Facilitator to join our team at the Nova Scotia Health Authority. In this role, you will provide psychosocial support and advance care planning services to patients, families, and caregivers living with advanced Chronic Obstructive Pulmonary Disease (COPD) enrolled in our INSPIRED COPD Outreach Program.

About You

To be successful in this role, you will have a graduate degree in Social Work or a related field, and relevant clinical experience. You will also possess current professional certification with a relevant professional body, such as the Nova Scotia Association of Social Workers (NSASW) or the Canadian Association for Spiritual Care (CASC). A minimum of 3 years of related experience is required, and experience working in the context of death and dying and/or palliative care is considered an asset.

Key Responsibilities
  • Provide psychosocial support and advance care planning services to patients, families, and caregivers living with advanced COPD
  • Collaborate with team members to develop individualized care plans and deliver, follow-up, and evaluate care based on these plans
  • Contribute to a culture of sensitive and supportive relationships with members of the INSPIRED team
  • Participate in team meetings and assist in the collection of quality assurance data as required
Requirements
  • Graduate degree in Social Work or a related field
  • Relevant clinical experience
  • Current professional certification with a relevant professional body
  • Minimum 3 years of related experience
  • Experience working in the context of death and dying and/or palliative care (asset)
