Healthcare Coordinator Specialist

Job Summary: Vancouver Coastal Health is seeking a skilled Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to fill the Coordinator role at Evergreen House in North Vancouver, BC. As a key member of our team, you will be responsible for providing high-quality patient care and contributing to the overall success of our healthcare facility.
Salary Range: CAD $49.20/Hr. - CAD $63.47/Hr., estimated annual salary range: CAD $101,388 - CAD $131,556 based on a 35 hour work week.
About the Role: In this position, you will have the opportunity to work with a diverse group of patients and colleagues, developing your skills and expertise as a healthcare professional. You will be responsible for coordinating patient care, collaborating with healthcare teams, and ensuring the highest level of patient satisfaction.
Key Responsibilities:
- Coordinate patient care activities, including assessments, treatments, and education
- Collaborate with interdisciplinary healthcare teams to develop and implement patient care plans
- Communicate effectively with patients, families, and healthcare professionals
- Maintain accurate and comprehensive patient records
Requirements:
- Diploma or degree in Nursing from an accredited institution
- Current registration with the College of Registered Nurses of British Columbia (CRNBC)
- 2+ years of experience in long-term care or a related field
- Strong communication and interpersonal skills
