Job Summary

We are seeking a highly skilled and compassionate Rehabilitation Assistant to join our team at Home and Community Care Support Services. As a key member of our healthcare team, you will provide essential support to our patients, working closely with Physiotherapists and Occupational Therapists to deliver high-quality care and services.

Key Responsibilities
  • Assist registered therapists with patient assessments and treatment programs, adhering to professional guidelines and standards.
  • Implement treatment plans with patients under the direction of registered therapists, utilizing Sensory Tech Platform and other organizational platforms as required.
  • Complete and maintain accurate documentation regarding patient interventions, adhering to Ontario Health atHome policies and procedures.
  • Meet with registered therapists regularly to discuss patient treatment plans and progress, ensuring seamless communication and collaboration.
  • Research and recommend assessment, diagnosis, and intervention tools, aids, and materials to support registered therapists in delivering patient services.
  • Promote maximum independence of patients in their homes, communities, and workplaces, supporting re-integration into society.
  • Contribute to a safe and healthy work environment by following safe work procedures, reporting injuries and illnesses, and addressing unsafe working conditions.
Requirements
  • Graduate of an accredited Physiotherapy Assistant/Occupational Therapist Assistant 2-year diploma program.
  • PTA/OTA certification recognized by the College of Physiotherapists of Ontario (CPO) and the College of Occupational Therapists of Ontario (COTO) required.
  • Previous experience as an Occupational Therapy Assistant/Physiotherapy Assistant in a healthcare setting within the last two years.
