Job Description:

Part-time Mental Health Social Worker Role

We are seeking a skilled and compassionate Mental Health Social Worker to join our dynamic team in the Mental Health Unit at North York General Hospital. As a key member of our inter-professional team, you will play a vital role in providing patient-centered care to individuals across the lifespan.

About the Position:

This is an exciting opportunity to work in a fast-paced environment where no two days are ever the same. You will have the chance to develop and implement comprehensive treatment plans, collaborate with community partners, and connect patients with essential resources.

Key Responsibilities:
  • Conduct biopsychosocial assessments to identify patient needs and develop personalized care plans
  • Liaise with community partners to ensure seamless transitions and support
  • Provide individual and group therapy modalities to promote emotional well-being
  • Offer counseling services to patients and families as needed
  • Maintain accurate records and communicate effectively with the healthcare team
Requirements:
  • Master's Degree in Social Work from an accredited institution
  • Registration with the Ontario College of Social Workers & Social Service Workers (OCSWSSW)
  • A minimum of one year of experience working in an acute care setting as a staff Social Worker
  • Geriatric experience is an asset
