Job Summary:

The Home and Community Care Program Manager plays a crucial role in ensuring the delivery of high-quality client care services. This individual is responsible for overseeing the management of staff, developing strategies to meet emerging opportunities, and collaborating with internal stakeholders to achieve department objectives.

Responsibilities:
  • Directs and monitors the delivery of client care services, ensuring compliance with regulatory guidelines and VON policies.
  • Provides guidance on risk assessments, investigations, and incident reporting.
  • Maintains performance metrics related to funder expectations and contributes to quality improvement initiatives.
  • Liaises with staff to facilitate orientation, training, and development.
  • Collaborates with internal stakeholders to develop team procedures and best practices.
  • Built strategic relationships with current, new, and prospective partners and community organizations.
  • Engages with clients and families to resolve complaints or issues related to service delivery.
Requirements:
  • Post-secondary education in a related field
  • Regulated Health Professional and certification as a Registered Nurse
  • A minimum of 5 years' experience in the functional area
  • A minimum of 2 years' experience managing others
  • Experience in community work and collaboration with community partners
Working Conditions:

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics.
