We are seeking a highly motivated and experienced Case Manager to join our team at Anishinabek Employment & Training Services. The successful candidate will be responsible for providing case management services to clients accessing an Alternate level of care (ALC), as well as coordinating and delivering cultural programming to Back to Home clients.

About the Role:

This is a challenging and rewarding role that requires strong communication and interpersonal skills. The successful candidate will work closely with other community partners to support the current development, treatment, cultural, and psycho-social needs of adults in the program.

Key Responsibilities:
  • Provide case management services to clients accessing an Alternate level of care (ALC)
  • Coordinate and deliver cultural programming to Back to Home clients
  • Work closely with other community partners to support the current development, treatment, cultural, and psycho-social needs of adults in the program
  • Ensure regular conferencing/reviews of relevant cases
  • Evaluate system effectiveness and recommend necessary changes

The ideal candidate will possess a strong understanding of Anishinabe history, culture, traditions, ceremonies, and values. A minimum three years' experience in case management with adults in the field of addictions and mental health is required. Extensive experience, training, and knowledge of Anishinabek culture and language are also essential.
