As a Healthcare Social Worker in our Home Health department, you'll play a vital role in providing comprehensive assessment, care planning, and treatment coordination for designated clients and their families.

About the Role

This is an exciting opportunity to join our dynamic team of healthcare professionals and make a real difference in the lives of our patients. As a Healthcare Social Worker, you'll work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, and group work, system navigation and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.

Key Responsibilities
  • Conduct comprehensive assessments of clients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address clients' specific needs and goals
  • Provide direct psychosocial care, counseling, and group work to support clients' mental health and well-being
  • Collaborate with other healthcare providers and the multidisciplinary team to ensure seamless care delivery
  • Facilitate access to community resources and services to support clients' independence and self-management
Requirements

To be successful in this role, you'll need:

  • A Bachelor's Degree in Social Work from an accredited university
  • Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders
  • Current full registration with the British Columbia College of Social Workers
  • Current valid driver's license
