Job Summary:

The Case Manager will work collaboratively with individuals, their families, and community agencies to develop and implement personalized plans to address their mental health needs and reduce the likelihood of further conflict with the law.

Key Responsibilities:
  • Conduct comprehensive assessments and develop individualized plans to address mental health needs and reduce recidivism
  • Collaborate with community agencies, healthcare providers, and court staff to ensure seamless service delivery
  • Provide education and support to individuals and their families on mental health, wellness, and recovery principles
  • Facilitate access to community resources, services, and support groups to promote recovery and independence
  • Monitor and evaluate the effectiveness of individualized plans and make adjustments as needed
Requirements:
  • Master's degree in a related field (e.g., social work, psychology, nursing)
  • Minimum 2 years of experience working in a community-based mental health setting
  • Knowledge of mental health reform principles, the Ontario Mental Health Act, and related legislation
  • Ability to work effectively with diverse populations, including youth and adults with mental health challenges
  • Excellent communication, interpersonal, and problem-solving skills
