Job Title: Case Manager - Mental Health and Justice

We are seeking a highly skilled and compassionate Case Manager to join our team in the Mental Health and Justice Program. As a Case Manager, you will work closely with adults and youth who have mental health challenges and are involved in the criminal justice system.

Key Responsibilities:
  • Collaborate with individuals to identify strengths and needs, and develop long-term goals and short-term objectives.
  • Assist individuals in acquiring skills and connecting with formal and informal support, peer support, professional services, and resources.
  • Navigate the court system and provide support through information, referral, and consultation.
  • Work directly with court staff to explore possible outcomes through the court process.
Requirements:
  • Degree or diploma in health or social services.
  • Experience in a community-based mental health setting, with experience with the criminal justice system an asset.
  • Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act.
  • Knowledge of systemic issues and social determinants of health.
  • Knowledge of the Ontario Mental Health Act and mental health reform principles.
Working Conditions:

Office work, community settings, meetings in clients' homes, and travel with clients.
