Job Title: Case Manager - Mental Health and Justice

Job Summary:

We are seeking a highly skilled and compassionate Case Manager to join our team in the Mental Health and Justice Program. The successful candidate will work with adults and youth who have mental health challenges and are involved in the criminal justice system.

Key Responsibilities:

  • Collaborate with individuals to identify strengths and needs, and develop long-term goals and short-term objectives.
  • Assist individuals in acquiring skills and connecting with formal and informal support, peer support, professional services, and resources.
  • Navigate the court system and provide support through information, referral, and consultation.
  • Work directly with court staff to explore possible outcomes through the court process.

Requirements:

  • Degree or diploma in health or social services.
  • Experience in a community-based mental-health setting, with experience with the criminal justice system an asset.
  • Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act.
  • Knowledge of systemic issues and social determinants of health.
  • Knowledge of the Ontario Mental Health Act and mental health reform principles.

Working Conditions:

Office work, community settings, meetings in clients' homes, and travel with clients.
