About the Role:
This is an exciting opportunity for a registered nurse or psychiatric nurse to work as a mental health case manager in our busy clinic. You will be responsible for providing high-quality patient care, conducting comprehensive assessments, developing treatment plans, and collaborating with multidisciplinary teams to ensure seamless care delivery.

Key Responsibilities:
- Conduct thorough mental health assessments to identify patient needs and develop individualized treatment plans

- Collaborate with interdisciplinary teams to coordinate care and services
- Develop and maintain relationships with community resources and healthcare providers to ensure comprehensive care delivery
- Stay up-to-date with best practices and evidence-based treatments in mental health
