Job Description:

We are seeking a Registered Nurse or Registered Psychiatric Nurse to join our team as a Mental Health & Addictions Case Coordinator. As a member of our multidisciplinary care team, you will provide treatment and case management services to individuals with mental health issues and concurrent disorders.

  • You will work collaboratively with clients, families, and other healthcare professionals to develop and implement individualized treatment plans.
  • Key duties include intake, mental health assessment, treatment planning, crisis intervention, consultation, individual counseling, education, and support groups.

Required Skills and Qualifications:

To be successful in this role, you will require:

  • A diploma in Nursing or Psychiatric Nursing
  • Two years' recent related experience providing treatment services to individuals with mental health issues
  • Broad knowledge of nursing theory and practice within a client-centred care model
  • Demonstrated ability to communicate effectively with clients and their families, colleagues, physicians, and other healthcare staff
