Job Description

We are seeking a highly skilled and experienced Home Care Program Case Coordinator to join our team. The successful candidate will be responsible for receiving referrals and conducting assessments to determine eligibility for Home Care program supports.

Key Responsibilities:
  • Receive referrals and conduct assessments to determine eligibility for Home Care program supports.
  • Develop, coordinate, and evaluate plans of care to support discharge from acute care settings.
  • Collaborate with clients, families, carers, and interdisciplinary team members to ensure comprehensive care planning.

Salary: $48,511 - $51,857 per year, depending on experience.

RequirementsEducation:
  • Graduate of an approved Registered Nursing education program required.
  • Bachelor of Nursing or Bachelor of Science in Mental Health preferred.
Experience:
  • Minimum three years recent related nursing experience required.
  • Experience with Hospital-Based Case Coordination an asset.
Certifications/Licenses:
  • Responsible for maintaining and providing proof of registration with the College of Registered Nurses of Manitoba (CRNM).
  • Cardiopulmonary Resuscitation (CPR) Training requirements for this position shall be in accordance with Employer policy.
Working Conditions

This is a term position with a duration of up to 1 year, with the possibility of extension based on performance and business needs.
