Stroke Rehabilitation Case Manager

We are seeking a highly skilled and dedicated Stroke Rehabilitation Case Manager to join our team at WRHA Community Health Services. As a valued member of our organization, you will play a crucial role in planning and coordinating stroke care services for clients in their homes and communities.
About the RoleThis is an exciting opportunity for a motivated and compassionate individual to work in a dynamic environment where no two days are ever the same. As a Stroke Rehabilitation Case Manager, you will be responsible for assessing client needs, developing personalized service plans, and coordinating the delivery of essential healthcare services.
Key Responsibilities:- Conduct comprehensive assessments of clients' physical, emotional, and social needs
- Develop and implement individualized service plans that address client goals and outcomes
- Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions
- Provide education and support to clients and their families on stroke recovery and rehabilitation strategies
- Maintain accurate records and reports to track client progress and service utilization

RequirementsTo succeed in this role, you will require:
- A minimum of two years of experience in acute or rehab hospital and/or community-based service delivery
- Previous experience in neurology, stroke care, and/or rehabilitation is strongly recommended
- Strong assessment and problem-solving skills, with the ability to work independently and collaboratively as part of an interprofessional team
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals
- Valid driver's license and access to a reliable vehicle (if applicable)
