Nursing Care Specialist

About the Role
As a Nursing Care Specialist, you will be responsible for providing direct care to clients, assessing their physical and emotional needs, and developing personalized care plans to meet their unique requirements. You will work closely with our team to ensure seamless coordination and communication, and will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Key Responsibilities
- Provide high-quality nursing care to clients in their own homes, including medication management, wound care, and vital sign monitoring.
- Assess clients' physical and emotional needs, and develop personalized care plans to meet their unique requirements.
- Collaborate with our multidisciplinary team to ensure comprehensive support and exceptional patient outcomes.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless coordination and communication.
Requirements
- Graduation from an accredited School of Nursing and current registration with the provincial licensing body.
- Valid CPR certification.
- At least two years of recent experience in a community setting; new grads may be considered for hire on an individual basis.
