About the Opportunity

Nova Scotia Health is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. You will work with interprofessional teams to facilitate hospital discharge and support clients in the community.

About You

We are looking for a candidate with a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. You should have current and ongoing registration with the applicable Nova Scotia Professional Regulatory body and a minimum of three years' experience. You should also have demonstrated knowledge and/or experience in a community or long-term care setting, as well as knowledge of community agencies and resources.

Hours

This is a permanent full-time position with 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays.

Compensation and Benefits

The successful candidate will receive a competitive salary of $41.06 - $46.75 hourly, as well as a comprehensive benefits package.
