About the Role

We are seeking a highly skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing high-quality patient care and support to individuals in their homes and communities.

Key Responsibilities
  • Assess and determine patient care needs and eligibility for physiotherapy services.
  • Provide access and referrals to community services and resources.
  • Engage with patients, caregivers, and other healthcare practitioners to ensure comprehensive care.
  • Develop and implement individualized treatment plans to meet patient needs.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making.
  • Participate in quality improvement initiatives to enhance patient outcomes and services.
Requirements
  • Master's degree in Physiotherapy or equivalent.
  • Current registration with the College of Physiotherapists of Ontario.
  • Minimum 2 years of experience in a physiotherapy role, preferably in a community-based setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Proficiency in Microsoft Office applications and electronic medical records.
  • Valid driver's license and access to a vehicle.
  • Fluency in English and French is required.
  • Vulnerable Sector Check completed within the last six months.
