Responsibilities- Provide nursing care to clients in their homes and communities, as per their individualized care plans.
- Conduct thorough assessments of clients' physical and emotional needs, and develop and implement care plans to meet those needs.
- Administer medications, perform wound care, and provide other nursing interventions as required.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Document client progress and care provided in a timely and accurate manner.
- Current licensure as a Licensed Practical Nurse (LPN) in the province of employment.
- Experience working in home care or visiting care agency settings, an asset.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to providing high-quality care.
- Valid driver's license and reliable vehicle, as travel to clients' homes is required.
