Mental Health Therapist

We are seeking a highly skilled and compassionate Mental Health Therapist to join our team at Homewood Health. As a Therapist for Mental Health and Addiction, you will play a vital role in delivering high-quality care to our clients and customers.
Key Responsibilities- Coordinate and conduct assessments to inform treatment planning, ensuring seamless services and programs for our clients and customers.
- Provide case management services, including service planning, coordination, navigation, and evaluation of progress, to support individuals struggling with complex mental health and/or addictions.
- Deliver individual and group psychotherapy utilizing evidence-informed practices, including Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, Brief Solution-focused Therapy, CBT, and DBT informed approaches.
- Offer individual and group support to family members and lead group counseling and education sessions, promoting a comprehensive understanding of mental health and addiction.
- Collaborate closely with Physicians and the clinical team to monitor patient experience, review progress, create and adjust treatment plans, and liaise with referral sources.
- Develop and implement discharge planning and referral to appropriate community resources, ensuring a smooth transition for clients.
- Identify discharge needs based on comprehensive patient assessments and care plans, prioritizing client-centered care.
- Provide service and team leadership by establishing treatment goals and working with patients and team members to develop action steps to meet these goals.
- Lead team monitoring and evaluation of progress on treatment plans, ensuring accountability and continuous improvement.
- Act as a communication lead among patients, the team, and program, ensuring critical information is communicated effectively.
- Complete documentation and record-keeping as per Homewood Health Centre and governing College policies, maintaining accurate and up-to-date records.
- Bachelor's or master's degree in a recognized Social Science program with addiction-specific education.
- Membership in good standing with the relevant regulatory college/governing body in Ontario or pursuit of relevant credential, OCSWSSW, COTO, CNO, or registration as a Registered Psychotherapist (RP) with the College of Registered Psychotherapists.
- Competence in providing CBT, DBT, and other evidence-based approaches for treating complex mental health conditions and substance use disorders.
- Demonstrated skills and experience in assessment and treatment, utilizing evidence-based practice.
- Exceptional communication and interpersonal skills, with the ability to facilitate and lead an interdisciplinary team.
- Awareness and acknowledgement of the value of using measurement-based care when supporting patients/clients to guide clinical decision-making, with experience an asset.
- Valid Ontario driver's license and access to own vehicle where required.
- Knowledge of the Mental Health Act, PHIPA, Ontarians with Disabilities Act 2005, and Health Care Consent Act.
