Forensic Social Work Assistant, Social Services Professional

Job Summary
We are seeking a highly organized and detail-oriented Social Services Coordinator to provide administrative support to our Forensic Social Work team. This is an exciting opportunity to work in a dynamic and fast-paced environment, supporting the delivery of high-quality patient care.
Key Responsibilities
- Provide clerical support to the social work department, including gathering charts, requesting and receiving information from the social work program and interdisciplinary teams.
- Liaise with staff to provide program-related information and represent the assigned program at various meetings.
- Schedule and book clinical appointments, prepare and maintain records, reports, and charts according to established procedures.
- Prepare and mail packages containing consent forms and questionnaires, monitor return of documents, and send reminders as required.
- Collect data from various sources, enter data into computerized systems, and ensure data is up-to-date.
- Set up and maintain patient and administrative filing systems, according to policies and procedures designed to protect confidentiality.
- Upload patient paperwork into the database, monitor and request missing documents from clinical staff.
- Provide administrative duties as required, including answering phone calls, faxes, emails, and general inquiries from the hospital and community.
Requirements
- Completion of a recognized Social Service Worker or Social Service Diploma Program or an equivalent combination of education, training, and experience.
- General knowledge of social work, mental health, and substance services.
- Ability to relate well with others and work effectively in a multidisciplinary team.
- Excellent communication and organizational skills.
- Ability to adjust to new or unexpected events, problem-solve, and effectively deal with conflict situations.
- Physical ability to carry out the duties of the position.
- Knowledge of medical terminology.
