Job Title: Physiotherapist, Ambulatory Programs

Baycrest Hospital is seeking a skilled Physiotherapist to join our Ambulatory Programs team. As a Physiotherapist, you will play a vital role in providing high-quality patient care and contributing to the development of our community-based programs.

Key Responsibilities:
  • Conduct comprehensive clinical assessments and develop personalized care plans for patients with complex geriatric, neurological, and musculoskeletal conditions.
  • Collaborate with inter-professional teams to implement client-focused care plans and coordinate discharge planning processes.
  • Provide education and support to patients, caregivers, and family members, as well as participate in the teaching and mentoring of physiotherapy and physiotherapy assistant students.
  • Assign patient plans of care and provide direction, supervision, and monitoring of physiotherapy assistants.
  • Support the development and implementation of evidence-based programs and contribute to the evaluation of program effectiveness.
Requirements:
  • Licensure in good standing with the College of Physiotherapists of Ontario.
  • Membership or eligibility for membership with the Canadian Physiotherapy Association (an asset).
  • Experience working with geriatric populations with multiple medical conditions and/or cognitive impairment (preferred).
  • Confirmed status as ADP authorizer required.
  • Expertise in clinical assessments of elderly clients.
  • Ability to lead groups and plan/executes knowledge translation/adult learning sessions with clients, caregivers, family members, and inter-professional team members.
  • Experience in providing clinical instruction and teaching to physiotherapy students (an asset).
  • Previous experience and/or interest in developing evidence-based programs (an asset).
  • Excellent interpersonal, communication, time management, and problem-solving skills.
  • Access to transportation (e.g., current driver's license and car) required.
  • Proficient computer skills – Meditech, MDS, Microsoft Word, PowerPoint, and Excel required.
