Job Summary

We are seeking a highly skilled Occupational Therapist to join our team as a Care Coordinator for Client Rehabilitation Services. This full-time position will involve working with a multidisciplinary team to provide high-quality care to clients.

About the Role

The successful candidate will be responsible for assessing client physical and cognitive functioning, creating individualized treatment plans, and collaborating with the inter-professional team at our clinic. Additionally, this role will involve supervising Occupational Therapy students (as preferred) and networking with stakeholders to promote Lifemark OT programs.

Key Responsibilities:
  • Conduct assessments of client physical and cognitive functioning
  • Create and implement individualized treatment plans based on analysis and clinical reasoning
  • Utilize e-charting software to document treatment sessions
  • Collaborate with the inter-professional team at our clinic
  • Supervise Occupational Therapy students (as preferred)
  • Network with stakeholders to promote Lifemark OT programs
Requirements

To be successful in this role, you will require a minimum of 3-5 years of experience treating clients with physical, neurological, and/or mental health conditions. New graduates are welcome to apply. You must also have demonstrated commitment to post-graduate professional development and a valid driver's license with access to a reliable vehicle.

Benefits

Lifemark Health Group offers a comprehensive compensation package, including an estimated salary of $80,000 - $110,000 per year, depending on experience.
