Vancouver Coastal Health is seeking a skilled Senior Rehabilitation Care Coordinator to join their team at GF Strong Rehabilitation Centre in Vancouver, BC. This role plays a vital part in facilitating care for patients and coordinating with the interprofessional team to ensure effective patient care.

Key Responsibilities
  • Facilitate comprehensive assessments and provide direction to the interdisciplinary team on the appropriate level of care
  • Collaborate with the acute interprofessional team to expedite effective patient care and coordinate timely sustainable discharge plans
  • Develop and recommend changes in policies, procedures, and standards to support effective care management and discharge planning
  • Provide assistance and support to clients and families in the transition from acute rehabilitation to the community

The successful candidate will possess a Master's degree in a related regulated Health Sciences discipline, along with three years' recent experience in rehabilitation care, including one year in case management and discharge planning. A current full registration with the applicable regulatory body is also required.

This position offers a competitive salary range of CAD $82,564 - CAD $102,818 per annum, depending on qualifications and experience. The estimated hourly rate is approximately CAD $39.67 per hour based on a 37.5-hour workweek.
