**Job Summary:** We are seeking a highly skilled Registered Practical Nurse to join our team as a Nurse Care Professional. This is an excellent opportunity for a nurse who wants to work in a supportive team environment with flexible schedules and competitive pay.

**Responsibilities:**

  • Complete comprehensive client assessments and develop care plans that meet their unique needs
  • Implement and evaluate care based on best evidence and practices
  • Support clients through care transitions by collaborating with other care team members and community support services

**Requirements:**

  • Current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification an asset
  • Enthusiasm and love of community care and passion for evidence-informed practice and excellence in care
