Job Summary
We are seeking a Registered Nurse to join our dynamic team. As a Nursing Professional with SE Health, you will have the opportunity to work with a diverse range of clients and contribute to their health and wellbeing. Your excellent communication and interpersonal skills, combined with your ability to work effectively in a fast-paced environment, make you an ideal candidate for this role.
Key Responsibilities
* Complete comprehensive client assessments to identify their unique needs and develop personalized care plans
* Implement and evaluate care based on best evidence and practices, working collaboratively with other healthcare professionals as needed
* Provide support to clients during care transitions, connecting them with community resources and services to ensure a smooth transition back to their homes
* Collaborate with the care team to develop and implement individualized care plans that meet each client's specific needs and goals
Requirements
* Current Certificate of Competence with the College of Nurses of Ontario
* Availability to work days, evenings, nights, and every other weekend
* Updated immunization records, clear criminal background check, and Vulnerable Sector check required upon hire
* Willingness to attain required SE Health certifications
* Current CPR certificate required
