Job Summary
Fraser Health is seeking a skilled Clinical Social Worker to join our Home Health team. As a member of our interdisciplinary team, you will provide assessment and treatment services to clients, liaise with community agencies, and participate in educational services, planning, and program development.
Key Responsibilities
- Provide therapeutic counseling services to clients and families experiencing complicated circumstances related to various medical diagnoses.
- Conduct social work and psychosocial assessments of clients and families, interpreting the information using various modalities in accordance with professional standards and clinical practices.
- Follow up and provide ongoing monitoring of complex psychosocial situations, evaluating clients' response to interventions.
- Participate in developing a comprehensive plan of care with/for the client, collaborating and consulting with other health service area professionals.
- Act as a resource for clients and their families, providing information regarding community resources and access to these resources, facilitating referrals where appropriate.
- Facilitate the development of HSA resources by identifying services needed to support the health of families.
- Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Promote client autonomy by ensuring their participation in the planning, decision-making, and development of problem-solving abilities.
- Assist in the development and provision of relevant educational programs for home health care staff, clients, their families, and community agencies on psychosocial issues related to health, illness, and disability.
- Prepare statistics, in accordance with the workload measurement system, for submission to designated personnel; maintain written clinical records as required.
Qualifications
- Master's Degree in Social Work from an approved school of Social Work.
- Two years' recent related experience in a complex health care environment such as a community or home health setting, or an equivalent combination of social work education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
