Clinical Social Worker, Home Health- New Westminster

As a Clinical Social Worker with Fraser Health, you will be working as a member of the Home Health interdisciplinary team to provide assessment and treatment services for clients. You will liaise with community agencies and caregiver support groups, participate in educational services, planning, program development, and policy recommendations. You will also provide consultative support to the interdisciplinary team in the development of services within the assigned area of responsibility.
Key Responsibilities
- Provides therapeutic counselling services to clients and families experiencing complicated circumstances related to a variety of medical diagnoses.
- Conducts social work and psychosocial assessment of clients and families and interprets the information using a variety of modalities in accordance with professional standards and clinical practices.
- Follows up and provides ongoing monitoring of complex psychosocial situations; evaluates clients' response to interventions.
- As a member of the interdisciplinary team, provides information regarding clients psychosocial situation and participates in developing a comprehensive plan of care with/for the client; collaborates and consults with other health service area professionals on the care plan.
- Acts as a resource for clients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate; facilitates the development of HSA resources by identifying services needed to support the health of families.
- Participates in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Promotes client autonomy by ensuring their participation in the planning, decision making, and the development of problem solving abilities.
- Assists in the development and provision of relevant educational programs for home health care staff, clients, their families, and community agencies on psychosocial issues related to health, illness, and disability.
- Prepares statistics, in accordance with the workload measurement system, for submission to designated personnel; maintains written clinical records as required.
- Performs other related duties as assigned.
Qualifications
- Master's Degree in Social Work from an approved school of Social Work.
- Two years' recent related experience in a complex health care environment such as a community or home health setting, or an equivalent combination of social work education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
