Job Title: Clinical Social Worker, Home Health

Job Summary:

We are seeking a skilled Clinical Social Worker to join our Home Health team in New Westminster. As a member of our interdisciplinary team, you will provide assessment and treatment services to clients, liaise with community agencies, and participate in educational services, planning, and program development.

Key Responsibilities:

  • Provide therapeutic counseling services to clients and families experiencing complicated circumstances related to medical diagnoses.
  • Conduct social work and psychosocial assessments of clients and families, interpreting information using various modalities in accordance with professional standards and clinical practices.
  • Follow up and provide ongoing monitoring of complex psychosocial situations, evaluating clients' response to interventions.
  • Collaborate with the interdisciplinary team to develop a comprehensive plan of care, providing information regarding clients' psychosocial situations and participating in care planning.
  • Act as a resource for clients and their families, providing information on community resources and facilitating referrals as needed.
  • Participate in quality improvement activities, providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
  • Promote client autonomy by ensuring their participation in planning, decision-making, and problem-solving abilities.
  • Assist in the development and provision of educational programs for home health care staff, clients, their families, and community agencies on psychosocial issues related to health, illness, and disability.
  • Prepare statistics for submission to designated personnel and maintain written clinical records as required.

Qualifications:

  • Master's Degree in Social Work from an approved school of Social Work.
  • Two years' recent related experience in a complex healthcare environment, such as a community or home health setting, or an equivalent combination of social work education, training, and experience.
  • Current full registration with the BC College of Social Workers.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
