Fraser Health is seeking a skilled Social Worker to join our team.
Job SummaryWe are looking for a highly motivated and experienced Social Worker to provide clinical and consultative services to patients and families. The successful candidate will conduct assessments, counseling, and planning for patient discharge, exploring and identifying required community resources.
Key Responsibilities- Conduct social work assessments to identify patient and family coping abilities and anticipated needs upon discharge.
- Complete written reports for inclusion in patient health records.
- Provide clinical counseling services to patients and families regarding identified problems, personal concerns, and treatment objectives.
- Consult and collaborate with interdisciplinary teams regarding patient care issues, resources, and discharge planning.
- Provide consultative and direct services to patients, families, and staff regarding community resources and access to these resources.
- Participate in department or program-sponsored research through literature searches, administering surveys, and analyzing data.
- Provide in-service education to staff through presentations and workshops.
- Participate in program and community resource development activities by working with community service agencies and the hospital.
The ideal candidate will possess a Bachelor's degree in Social Work from an approved school of Social Work, plus one year of recent related clinical social work experience in a healthcare setting. Current full registration with the BC College of Social Workers is also required.
The successful candidate will have knowledge and understanding of psycho-emotional and social issues related to illness/injury/disease and current social work treatments. They will also possess strong communication and interpersonal skills, with the ability to work effectively with patients, healthcare teams, and external stakeholders.
