Clinical Social Worker, Home Health- New Westminster

Job Summary
We are seeking a skilled Clinical Social Worker to join our Home Health team at Fraser Health. As a member of our interdisciplinary team, you will provide assessment and treatment services to clients, liaise with community agencies, and participate in educational services, planning, and program development.
Key Responsibilities
- Provide therapeutic counselling services to clients and families experiencing complicated circumstances related to medical diagnoses.
- Conduct social work and psychosocial assessments of clients and families, interpreting information using various modalities in accordance with professional standards and clinical practices.
- Follow up and provide ongoing monitoring of complex psychosocial situations, evaluating clients' response to interventions.
- Collaborate with the interdisciplinary team to develop a comprehensive plan of care, providing information regarding clients' psychosocial situations and participating in care planning.
- Act as a resource for clients and their families, providing information on community resources and facilitating referrals where appropriate.
- Participate in quality improvement activities, providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Promote client autonomy by ensuring their participation in planning, decision-making, and problem-solving abilities.
- Assist in the development and provision of educational programs for home health care staff, clients, their families, and community agencies on psychosocial issues related to health, illness, and disability.
- Prepare statistics for submission to designated personnel and maintain written clinical records as required.
Qualifications
- Master's Degree in Social Work from an approved school of Social Work.
- Two years' recent related experience in a complex healthcare environment, such as a community or home health setting, or an equivalent combination of social work education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
