About this Role:

The Home Health Registered Nurse Liaison plays a vital role in ensuring seamless transitions of patients from acute care to community-based services. This dynamic position requires a skilled nurse to assess client needs, facilitate discharge planning, and coordinate care with community service providers.

Key Responsibilities:

- Arrange client referrals to community care services upon discharge or change in level of service
- Conduct comprehensive client assessments to determine appropriate care plan
- Develop and implement client care plans with interdisciplinary teams
- Participate in discharge planning rounds to ensure timely patient discharge
- Liaise with clients, families, physicians, and healthcare professionals to arrange community services
- Facilitate information exchange between acute care facilities and community services
- Determine client eligibility for community services and placements
- Maintain accurate documentation of client information

Requirements:

- Graduation from an approved school of nursing
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
- One year recent related clinical experience in acute care and/or community nursing and discharge planning
- Ability to communicate effectively, problem-solve, and prioritize client caseloads
- Knowledge of acute, chronic, rehabilitation, and palliative health conditions, as well as broad healthcare services and community resources
