Community Care Liaison

Fraser Health is seeking a highly skilled Social Work Liaison to join our team. In this role, you will play a critical part in facilitating the discharge process for patients from acute care facilities to community-based services.
Main Responsibilities:
Conduct client assessments and work with interdisciplinary teams to develop comprehensive care plans.
Collaborate with community service providers to arrange referrals and coordinate care transitions.
Advise acute care staff on client care needs and provide guidance on community resources.
Manage short-term Home Support hours and ensure timely discharge of patients.
Participate in quality improvement activities and contribute to the development of indicators and standards.
Requirements:
- Bachelor's degree in Social Work from an approved school.
- Two years' recent related clinical experience working with geriatric patients and adults with chronic illnesses.
- Current full registration with the BC College of Social Workers.
Skills and Abilities:
- Knowledge of the health system, community resources, and volunteer agencies.
- Demonstrated ability to deal with others effectively.
- Ability to communicate effectively both verbally and in writing.
