Role Overview
As a key member of our interdisciplinary team at Fraser Health, we are seeking a skilled Social Worker to join our Renal Program. In this role, you will provide essential psycho-social assessments and interventions to clients and their families, helping them navigate the challenges of renal disease and its treatment.
- Assessment and Intervention
Conduct comprehensive psycho-social assessments to identify the unique needs and concerns of clients and their families. Develop and implement personalized treatment plans to address their physical, emotional, and social well-being.
Counselling and EducationProvide supportive counselling services to clients and their loved ones, helping them cope with the impact of renal disease on their daily lives. Offer education and guidance on lifestyle changes, treatment objectives, and access to community resources.
Community ConnectionsConnect clients and their families with relevant community and rehabilitation services, facilitating their access to financial, vocational, transportation, childcare, and accommodation support.
Interdisciplinary CollaborationParticipate in developing comprehensive plans of care with our interdisciplinary team, ensuring a seamless and coordinated approach to client care.
Requirements
- Education and Experience
Bachelor's degree in Social Work from an approved school of Social Work. Two years' recent related clinical experience working with clients in an acute care and/or community healthcare environment.
Registration and CertificationCurrent full registration with the BC College of Social Workers.
Skills and AbilitiesComprehensive knowledge of systems theory. Ability to apply systems theory to practice, social work assessment, and intervention. Strong communication and interpersonal skills, with the ability to lead and collaborate within an interdisciplinary setting.
