Job Title: Social Worker, Renal

Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Renal Program team. As a key member of our interdisciplinary team, you will provide psycho-social assessments and treatment plans for clients and families in the Renal Program, promoting their quality of life and well-being.

Key Responsibilities

  • Conduct psycho-social assessments and develop treatment plans to address the unique needs of clients and families in the Renal Program.
  • Provide clinical and supportive counseling services to clients and their significant others, addressing issues related to illness, adjustment, and lifestyle changes.
  • Collaborate with healthcare professionals to provide education and support services to individuals, couples, families, or groups, promoting their understanding of the psycho-social implications of treatment modalities.
  • Organize and coordinate peer support programs for assigned clients, fostering a sense of community and connection.
  • Consult with clients, families, and staff to provide information and resources related to community and rehabilitation services, addressing financial, vocational, transportation, child care, and accommodation concerns.
  • Participate in quality improvement activities, contributing to the development, implementation, and evaluation of departmental and program goals, objectives, policies, and procedures.
  • Prepare statistics and reports, as required, to support the workload measurement system.
  • Participate in department or program-sponsored research, contributing to literature searches, survey administration, and data analysis.

Qualifications

  • Bachelor's degree in Social Work from an approved school of Social Work.
  • Two years' recent related clinical experience working with clients in an acute care and/or community health care environment, or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
