Job Summary:


This role is responsible for the management of specific cases as assigned, coordinating and supporting patients through their journey. The Bilingual Nurse Case Manager (NCM) oversees patient cases and serves as the primary point of contact for assigned healthcare providers (HCP). Additionally, this role focuses on driving positive customer and patient experience in a fast-paced environment.


This position's patient case load will reside in the Atlantic territory, requiring flexibility in working rotational or varying hours (8 am to 8 pm EST Monday to Friday).



Key Responsibilities:



  • Assist prescribing physicians in completing patient enrolment, gathering necessary information for private and provincial insurance forms in a concise, organized manner.
  • Facilitate communication with physicians, Patient Support Programs, or other HCP involved in patients' care circles.
  • Work collaboratively with team members to coordinate patient journeys.
  • Coordinate document submissions and cases with insurance companies, physician offices, and healthcare professionals to assist patients.
  • Provide educational guidance to patients and physicians on product administration.
  • Serve as a central resource liaison for program stakeholders, including patients, HCPs, clinics, and pharmacies.
  • Maintain accurate patient records to meet program reporting requirements.
  • Identify and report adverse events.
  • Answer general inquiry calls via telephone or web-based platforms regarding program services, disease states, or product-related questions based on program guidelines.
  • Understand the reimbursement landscape and ensure correct documents are submitted to insurers for a positive outcome in a timely manner.
  • Build relationships with key customers within their area of responsibility to establish business requirements and portfolio management.
  • Conduct field visits to support HCPs with paperwork according to program requirements.
  • Able to work various shifts, including days, evenings, weekends, and holidays as required.
  • Perform other duties as assigned.


Requirements and Qualifications:



  • Practicing Registered Nurse (RN), Licensed Practical Nurse (LPN/RPN) with good standing membership in respective regulatory colleges.
  • Bilingual in English and French (written and verbal).
  • Reimbursement knowledge and understanding is an asset.
  • 1-3 years of experience in a relevant role.
  • Exceptional customer service and interpersonal skills.
  • Ability to manage multiple tasks and priorities simultaneously.
  • Strong analytical and organizational skills.
  • Effective issue resolution and critical thinking abilities.
  • Excellent time management skills and sense of urgency.
  • Detail-oriented and results-driven.
  • Adaptability to change and ambiguity.
  • Hold a valid, unrestricted driver's license and possess a car.
  • Adhere to Performance Standards and Procedures required by the program.
  • Maintain a private, dedicated home office space accommodating equipment.
  • Flexibility in working rotational or varying hours (8 am to 8 pm EST Monday to Friday).
  • Preferably based in the Atlantic region but not mandatory.

Estimated Salary Range:


The estimated salary range for this position is $47,400 - $79,000 per year, based on performance, experience, and location.
