About the Role:

CarePartners is seeking a skilled Occupational Therapist to join our dynamic Rehabilitation Team. As an Occupational Therapist, you will play a critical role in providing high-quality patient care and promoting independence in the home and community.

Key Responsibilities:

  • Provide professional assessment of patients in the community and in their homes
  • Develop and maintain effective Care Plans with respect to Community Health Services
  • Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team
  • Provide one-on-one therapeutic support with clients and promote independence in the home and community
  • Collaborate with the Community Support Workers in the field and provide coaching and supervision as needed

Requirements:

  • Master's Degree in Occupational Therapy from a recognized university
  • Registration with the College of Occupational Therists of Ontario
  • Proven computer skills, including email, MS Word, and Outlook, work scheduling systems
  • Strong assessment skills and the ability to participate in effective treatment programs
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Clear Background and Vulnerable Sector Check
