Job Summary
Home and Community Care Support Services is seeking a skilled Occupational Therapist to provide high-quality care to patients in their homes and communities.
Key Responsibilities
- Provide occupational therapy services to patients to maximize their function, independence, and safety in their environment.
- Assess patients' needs and develop personalized care plans to meet their goals.
- Collaborate with healthcare professionals, family members, and caregivers to ensure comprehensive care.
- Stay up-to-date with the latest occupational therapy tools, processes, and equipment to provide best-in-class care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
- Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
- Strong knowledge of occupational therapy services and community resources.
- Excellent communication and interpersonal skills to work with diverse patient groups and healthcare teams.
- Ability to prioritize duties, manage multiple patients, and efficiently organize workload.
- Valid driver's license and access to a vehicle.
- Advanced oral and written proficiency in English and French.
