Job Title: Occupational Therapist
Job Summary:
We are seeking a highly skilled Occupational Therapist to join our team at Home and Community Care Support Services. As an Occupational Therapist, you will be responsible for providing occupational therapy services to patients in their homes and communities.
Key Responsibilities:
- Provide occupational therapy services to patients in their homes and communities
- Assess patients' needs and develop individualized treatment plans
- Collaborate with other healthcare professionals to ensure comprehensive care
- Provide education and support to patients and their families
- Conduct home visits and assessments to identify patients' needs
- Develop and implement treatment plans to improve patients' function and independence
- Monitor patients' progress and adjust treatment plans as needed
Requirements:
- Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario
- Experience working in a community or healthcare environment
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and access to a vehicle
