About the Role

This critical position impacts the lives of our residents and fosters meaningful connections within our communities. Your day will be filled with:

  • Leading resident move-in, transfer, and move-out processes, ensuring seamless transitions and warm welcomes.
  • Providing social services counseling as required, leveraging your qualifications to support residents' needs.
  • Developing and implementing individualized social work services and support groups to address resident and family concerns.
  • Supporting advanced care planning and treatment conversations, including funeral arrangement discussions.
  • Responding to feedback from residents and families, ensuring timely resolutions and satisfactory outcomes.
  • Collaborating with departmental leaders to provide coaching and direction to resident home area teams, driving service excellence and hospitality.
Requirements

To succeed in this role, you'll need:

  • Social Services Worker registration per provincial legislation.
  • A minimum of three years' experience in healthcare, social services, or a related sector.
  • Knowledge of the Long Term Care Act & Regulations is preferred.
  • Strong customer service and communication skills, with the ability to organize, plan, and delegate responsibilities.
