About the Role

We are seeking a highly skilled and dedicated Field Case Manager to join our team at CB Canada. As a Field Case Manager, you will play a critical role in managing patient cases and providing exceptional customer service to our healthcare partners.

Key Responsibilities
  • Case Management: Manage patient cases from intake to resolution, ensuring timely and effective resolution of patient needs.
  • Communication: Develop and maintain strong relationships with healthcare providers, patients, and internal stakeholders through effective communication and collaboration.
  • Program Coordination: Coordinate program activities, including patient enrollment, data collection, and reporting, to ensure seamless program execution.
  • Problem-Solving: Identify and resolve complex patient cases, utilizing clinical knowledge and problem-solving skills to ensure optimal patient outcomes.
  • Continuous Improvement: Participate in continuous quality improvement initiatives to enhance program effectiveness and patient satisfaction.
Requirements
  • Education: Bachelor's degree in a healthcare-related field or equivalent experience.
  • Experience: Minimum 2 years of experience in a healthcare-related field, preferably in case management or a related role.
  • Skills: Excellent communication and interpersonal skills, strong analytical and problem-solving skills, and ability to work in a fast-paced environment.
  • Certifications: Current certification in a relevant healthcare field, such as CCM or ACM, is an asset.
