Program Manager Job Description

Spectrum Health Care Inc is a renowned healthcare organization with over 40 years of experience delivering innovative home health care and specialized services.

We are currently seeking an exceptional candidate to fill the position of Program Manager, Integrated Specialty Programs for Palliative Care.

This role offers a unique opportunity to lead a diverse, multidisciplinary team in delivering high-quality, integrated care in the community. As a Program Manager, you will be responsible for driving operational and clinical excellence while cultivating and managing high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals.

About the Role
  • Leverage data-driven insights to optimize service delivery, enhance quality, and achieve key outcomes.
  • Collaborate with external health system stakeholders to support the design and implementation of innovative programs.
  • Foster a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.
Responsibilities
  • Lead program development and design clinical workflows to support innovative models of care.
  • Recruit and onboard team members, ensuring they have the necessary attributes, training, and skills.
  • Lead and mentor the team to consistently meet high care quality standards.
  • Address quality, safety, and risk issues, and develop improvement plans as needed.
  • Monitor and report key performance indicators, initiating improvements or process changes when required.
Requirements
  • A minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager, preferably in a community setting.
  • Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or Fundamentals of Palliative Care; or The Comprehensive Advanced Palliative Care Education (CAPCE).
  • Experience managing in a union environment is an asset.
Compensation and Benefits

The estimated annual salary for this position is approximately $85,000 - $110,000, depending on qualifications and experience.
