Healthcare Leadership Professional for Long-Term Care

Compensation Package: We offer an attractive compensation package, including a salary range of $120,000 - $160,000 per annum, based on experience.
About the Role: As Associate Director, Asset Management, you will be responsible for overseeing clinical care processes, ensuring that care is delivered according to individual plans of care for residents. You will work closely with hospital staff, doctors, external providers, and interdisciplinary services to ensure effective provision of care delivery in the home.
Main Responsibilities:
- Clinical Care Oversight: Plan, direct, develop, coordinate, implement, and evaluate resident care services; oversee clinical care processes to ensure that care is delivered according to individual plans of care for residents.
- Care Coordination: Liaise with hospital, doctors, external providers, and available interdisciplinary services to ensure the most effective provision of care delivery in the home.
- Program Implementation: Lead the implementation and monitoring of required clinical programs, i.e., Falls Prevention, Skin and Wound Care, Continence Care, Pain Management.
- Resident Care Plans: Oversee the development, implementation, review, and evaluation of resident care plans; ensure all RAI-MDS assessments are completed on time, all care conferences are held, and information is captured for discussion and ensures all data is ready for submission in partnership with the RAI coordinator and CIHI.
Operational Experience: Ensure quality care and a safe, effective, and efficient work environment. Assume responsibilities of the Director of Care as required in their absence. Promote resident and family satisfaction and continuous quality improvement initiatives.
Human Resources: With support of the Director of Care, hire, direct, supervise, evaluate, and performance manage personal support workers and registered team members. Monitor performance, conduct performance appraisals, and work with the Director of Care to resolve performance concerns.
Customer Service: Model and foster an environment of positive customer service in all aspects of care. Facilitate interdisciplinary care conferences with the assistance of the Social Worker(s), with special attention to resident/attendance. Develop strong relationships with residents/families, becoming a trusted member of the circle of care; ensure professional relationships are fostered with all team members and residents/families.
Qualifications:
- Bachelor's degree in Nursing or related science discipline
- Masters in Nursing or Healthcare an asset
- Current Registration as a RN with College of Nurses Ontario
- One year experience as RN in long term care is required.
- One year experience in supervisory role in long term care is preferred.
- Three years experience as RN in supervisory or managerial role in health care is required.
- Possession of strong motivational, team building and time management skills.
