Psychiatric Liaison Nurse

Fraser Health is seeking a skilled Psychiatric Liaison Nurse to join our team. As a key member of our interprofessional care team, you will provide person-centered and recovery-oriented care to patients with psychiatric and concurrent disorders.
Key Responsibilities- Perform intake assessments and offer crisis intervention to patients in the Emergency Department
- Provide input into the care of psychiatric patients awaiting transfer or inpatient admission
- Collaborate with the interprofessional care team to ensure effective patient flow and adapt care plans to address psychiatric needs
- Communicate and consult with Coordinators and other members of the interprofessional mental health and substance use team on patient care issues, resources, and discharge planning
- Document assessments, care plans, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures
- Graduation from an approved school of Nursing or Psychiatric Nursing
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- One year recent related experience in acute or community mental health and substance use setting or an equivalent combination of education, training, and experience
