About This Role

At Fraser Health, we are seeking a skilled Mental Health Liaison Nurse Specialist to join our team. As a key member of our interprofessional care team, you will play a vital role in providing high-quality patient care and promoting recovery-oriented services.

As a Psychiatric Liaison Nurse, you will be responsible for performing intake assessments, offering crisis intervention, and providing input into the care of psychiatric and concurrent disordered patients in the Emergency Department. You will also work closely with the interprofessional community mental health and substance use team to ensure effective patient flow and adapt care plans to address psychiatric needs.

This is a part-time opportunity based in Langley, where you will have the chance to make a meaningful difference in the lives of our patients and families.

Key Responsibilities
  • Screen psychiatric referrals from various internal and external sources, gathering background information and determining service priorities.
  • Perform psychosocial assessments and collect collateral related to patients' primary support group, social environment, occupation, education, housing, finance, health service access, and legal problems.
  • Develop care plans with final dispositions authorized by the Emergency Room Physician or consulting psychiatrist, and provide general and crisis counselling to patients and their families.
  • Communicate and consult with Coordinators and other members of the interprofessional mental health and substance use team on patient care issues, resources, and discharge planning.
  • Document assessments, care plan, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
Requirements
  • Graduation from an approved school of Nursing or Psychiatric Nursing.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year recent related experience in acute or community mental health and substance use setting or an equivalent combination of education, training, and experience.
