Job Description

This position is part of the Nephrology Program at Humber River Health, a dynamic and innovative healthcare organization.

About the Role

The Dialysis Liaison is an integral member of our team, responsible for providing home haemodialysis treatment in the community within our catchment area. This role requires strong communication skills, attention to detail, and the ability to work independently with minimal supervision.

Responsibilities:
  • Assist patients with home hemodialysis treatments, ensuring their quality of life is maintained according to treatment plans.
  • Maintain accurate records and documentation related to patient care.
  • Continuously observe patients and report status changes to registered nurses.
  • Ensure correct use of equipment and supplies, reporting any issues or malfunctions.
Requirements:
  • Personal Support Worker certificate or equivalent.
  • Minimum 1 year experience in a related field.
  • Valid driver's license and reliable vehicle for travel within HRH's community.
  • Current BCLS & CPR Certificates.
  • Effective written and oral communication skills, as well as interpersonal skills for working within a multidisciplinary team.
